We have been informed of changes made regarding the New York State requirement for health examinations for school attendance.

Beginning with the 2018-2019 school year students who will be in the listed grades will be required to have a physical exam:

  • Pre-K or Kindergarten
  • First Grade
  • Third Grade
  • Fifth Grade
  • Seventh Grade
  • Also, if participating in middle school sports, an exam is required.

Physical must be done within a year of the first day of school. Since school begins on September 5, 2018 a physical done after September 2017 is acceptable for 2018-19 school year.

Attached is the new NYS Health Examination Form. Computerized forms from your physician will be accepted next year but not in the 2019-2020 school year.

Immunization requirements have not changed. Kindergarten students are still required to have two MMR’s and two Varicella’s as well as a polio vaccine given after the age of four. Sixth grade students need a Tdap Vaccine and two Varicella’s and Seventh grade students need a Meningococcal vaccine. All vaccines need to be given before school begins on September 5, 2018. Proper documentation is required. All forms must be legible, signed and stamped by the doctor.
